When we talk about hair loss, the medical term Alopecia encompasses many different scenarios. Here I will explain, in simple terms, the main types, their causes, and why it's important to identify which type you have.
1. Androgenetic alopecia (common baldness): This is the most common type in both men and women. In men, the hairline recedes, and the crown thins; in women, the loss is more diffuse over the vertex. This form has a genetic and hormonal origin: a testosterone metabolite called DHT shortens the hair follicle's growth cycle.
2. Alopecia areata (sudden patches): Here, an autoimmune reaction attacks the hair follicles, causing round, hairless patches that can appear on the scalp, eyebrows, or other body parts. It can progress to more complete forms like Alopecia totalis (entire scalp) or Alopecia universalis (entire body).
3. Telogen effluvium (rapid diffuse shedding): Rather than patches, there is general thinning or abundant hair shedding due to triggering factors: fever, surgery, stress, hormonal changes, nutritional deficiencies. Here, follicles enter a resting phase and fall out simultaneously.
4. Traction alopecia (damage from tension): Caused by very tight hairstyles, extensions, heat, or aggressive products. Repeated effects damage the follicles; if it progresses too much, it can lead to permanent areas of loss.
5. Cicatricial or scarring alopecia: Here, the loss is permanent because the follicle is destroyed and replaced by scar tissue. There may be prior redness, itching, or pain.
Why is it crucial to recognize the type you have?
Because treatment and the possibility of recovery depend on the mechanism: genetic, immunological, mechanical, or due to irreversible inflammation. Some alopecias can be stopped or improved, while others require early intervention to prevent permanent damage.
